General Function
Under the supervision of the Program Manager or Director, the Youth Development Specialist supervises and mentors Youth Ambassadors (ages 16–18), providing opportunities for youth to build job readiness, leadership, and life skills while strengthening connections with their peers and community. The Youth Development Specialist facilitates wellness groups, supports program events and activities, and provides individualized relational support and case management for Youth Ambassadors aligned with the program logic model. The Youth Development Specialist also supports program evaluation in partnership with Youth Ambassadors.
In addition, the Youth Development Specialist serves as a Lead Care Manager (LCM) and provides Enhanced Care Management (ECM) services for Youth Ambassadors and other assigned clients. In this role, the staff member coordinates client-centered care by helping individuals navigate medical, behavioral health, and social service systems. The Lead Care Manager works with clients experiencing complex or chronic needs to improve health outcomes, strengthen service coordination, and connect clients to community resources. Through relationship-based support and collaborative care planning, the Lead Care Manager helps clients build a network of services that empowers them to navigate their care needs and improve overall wellbeing and quality of life.
Primary Responsibilities
Youth Ambassador Facilitation
- Supervise and mentor Youth Ambassadors through weekly 1:1 meetings, goal setting, coaching, and leadership and workforce skills development.
- Facilitate wellness groups, leadership activities, and cultural skill-building that support youth development and peer-to-peer mentorship.
- Provide relational support and case management for Youth Ambassadors, supporting their personal, educational, and employment goals.
- Support youth participation in program events, work opportunities, and community engagement activities.
- Safely transport youth to and from programming, work opportunities, and events throughout Humboldt County.
- Build and maintain relationships with cultural consultants, elders, and community partners who support youth programming.
- Maintain required program documentation, reporting, and data collection related to youth participation and program outcomes.
- Attend Leadership Program meetings, staff meetings, trainings, and regular supervision.
- Maintain a flexible schedule that will include evenings, weekends, and occasional overnight travel for youth trips and events.
Enhanced Care Management (Lead Care Manager)
- Provide Enhanced Care Management (ECM) services for Youth Ambassadors and other assigned clients with complex health, behavioral health, or social needs.
- Engage eligible clients in ECM services using relationship-based outreach and multiple engagement strategies.
- Conduct assessments to identify medical, behavioral health, housing, and social service needs.
- Develop, implement, and monitor individualized care plans in collaboration with clients, families, providers, and community partners.
- Serve as the primary point of contact and lead coordinator for the client’s multidisciplinary care team.
- Coordinate care across medical, behavioral health, and social service providers to ensure integrated and client-centered services.
- Connect clients to resources including mental health care, physical health services, substance use treatment, housing, food access, and other social supports.
- Advocate for clients to ensure their needs, preferences, and cultural context are reflected in care planning and service delivery.
- Facilitate care team communication and case coordination meetings when needed to support client progress.
- Provide services in community-based settings where clients live, work, or spend time.
- Document services and maintain accurate case records in the agency’s Electronic Health Record within required timelines and in compliance with agency, contractual, state, and federal requirements.
Outreach & Community Engagement
- Develop positive relationships with youth and community members in schools and community settings.
- Facilitate groups and programming at school sites and in community locations as assigned.
- Support planning and implementation of Two Feathers events, workshops, and youth programming.
- Participate in community outreach efforts including tabling, presentations, and information sharing about Two Feathers services.
- Collaborate with other departments and community partners to support shared youth and
coordinated services.
Qualifications
- Experience: At least two years prior experience working with youth.
- Education: High school diploma or equivalent.
- Current driver’s license, in good standing.
- Fingerprint clearance with the Department of Justice, FBI, and Child Abuse Index.
- Must comply with a drug-free workplace.

Work Conditions
- Lifting and carrying up to 30 pounds; pushing and pulling up to 50 pounds
- Stand for extended periods of time
- Sit for extended periods at a computer and driving in a vehicle.
- Ability to drive distances for up to 2 hours at one time.
- Must be available to work nights and weekends, as well as occasional overnight trips
